The Poisoning of Michigan fiasco was made into a movie starring Dennis Weaver. Audubon (I think) had a really good -- and really scary -- writeup a couple/few years after the incident happened. In that case, it was a mixup in bagging the fire-retardant (including PCBs) in distinctive-looking feed bags after the factory ran out of fire retardant bags.
