Sal,
They need to fix that form at the above address. They seem to have
forgotten that residents of Puerto Rico and the US Virgin Islands are also
US Citizens and a few of us may want to participate. But we can't as long
as they require a full address including State and leave us off the list.
This is not a form where you fill in the state name or postal code for the
state, but one in which they list (supposedly) all the states and you
choose one.
PR is not listed and the form bombs when I try to send it. Also missing
are USVI and Guam.
I will attempt to contact them direct, but just in case....
